One of my favorite 'Add Ons' in Google Docs is Doc to Form. This take a document and transforms it to a Google Form. This is an excellent tool on the SAMR model.
You are moving a 'Document' to a Google Form. This is augmenting or modifying the task. Google Forms give you a lot of flexibility in transforming how information is both presented and how you receive the data. Think about it: you can create a form that allows for embedded intervention supports when a student answers a question wrong. They can see how they missed it and do review right then and there.
